# Ozempic Click-Counting: Measurement and Math Audit

As of 2026-07-14. Scope: unofficial click-counting claims for Ozempic pens, with a focus on the U.S. 4 mg/3 mL pen that officially delivers 1 mg doses.

## Overall assessment: share with explicit caveats

The institutional click tables are valid evidence that the practice exists. They are **not** mutually consistent measurements of delivered dose. The corrected reporting frame is “competing published unofficial recipes,” not “a validated conversion range.”

## Material findings

1. **The 72–74 notation is source disagreement, not uncertainty.** BC sources use a 72-click anchor; the ADA journal report uses 74. No source supplies a confidence interval, engineering tolerance, or measured accuracy for unmarked settings.
2. **One hospital handout is internally inconsistent.** BC Children’s says both “1 click = 0.01 mg” and “72 clicks = 1 mg.” The first statement would make 72 clicks equal 0.72 mg. These are the ADA authors’ mathematically calculated, manufacturer-unconfirmed examples:

The BC sources instead publish 18/36/72 for approximately 0.25/0.5/1 mg on an Ozempic 1-mg-dose pen. ## Independent proportional checks for the 4 mg/3 mL, 1-mg-dose pen

Assumption used only for sensitivity analysis:

The full-scale source disagreement is approximately 2.7–2.8%. This is an arithmetic consequence of the anchors, not a measured error rate.

### One-click sensitivity

Under the same assumptions, one click is approximately:

### Whole-click and displayed-dose rounding

Click charts contain a second arithmetic choice beyond the 72-versus-74 source anchor: the dial is described in whole clicks, while a proportional target can land between clicks.

- On a 74-click scale, 0.25 mg corresponds to exactly 18.5 clicks. The ADA table prints 18, but does not state a general floor/ceiling/half-tie rule.
- If nearest-integer rounding is assumed, rounding alone can contribute up to ±0.5 click. If a source instead always rounds down or up, the magnitude and direction change.
- Some displayed milligram values are rounded too. For example, 49/74 = 0.6622 mg, displayed as 0.66 mg; 59/74 = 0.7973 mg, displayed as 0.8 mg; and 51/72 = 0.7083 mg, displayed as 0.71 mg.

These are rounding effects inside the source models. They are separate from source disagreement, device tolerance, injection loss, and biological response.

## Why concentration arithmetic does not validate the charts

- Nominal contents: 4 mg ÷ 3 mL = 1.3333 mg/mL.
- Printed label concentration: 1.34 mg/mL.
- Printed concentration × total volume: 1.34 × 3 = 4.02 mg.
- EMA printed full-dose volume: 0.74 mL.
- Printed concentration × printed full-dose volume: 1.34 × 0.74 = 0.9916 mg.

These differences are consistent with rounded regulatory display values. They are not evidence of a precise 0.02-mg excess, exact overfill, or an exact dose per click.

If one nevertheless assumes 0.01 mL per click, the rounded 1.34 mg/mL concentration produces 0.0134 mg per click and 74.63 clicks per 1 mg. That does not reproduce 72, 74, or 75 exactly. ## Required device-identification fields

Before comparing or reproducing any click claim, capture:

- country and market;
- manufacturer and brand;
- exact product code, NDC, or DIN;
- total milligrams and total millilitres;
- printed concentration;
- marked selectable dose or doses;
- pen/device version and date;
- whether clicks were counted forward from zero, after a flow check, or during backward correction.

Without these fields, a click chart is not reproducible.

## Delivery and residual-volume considerations

- Flow check: performed before first use and repeatable if no drop appears; it expels an unspecified amount and complicates leftover arithmetic.
- Direction: the FDA says the selector clicks differently forward and backward.
- Blockage: the counter may move even when a blocked needle delivers no medicine.
- Hold time: removing the needle before six seconds may underdeliver.
- Needle storage: leaving the needle attached can cause leakage, blockage, contamination, and inaccurate dosing.
- Residual solution: the official instructions say to use a new pen if the counter cannot reach the full marked dose; visible liquid is not a defined extra dose.

## Storage and cost arithmetic

Pure content division gives 4, 8, 16, or 32 theoretical doses from a 4-mg-total pen at 1, 0.5, 0.25, or 0.125 mg per week. The U.S. label, however, requires disposal after 56 days even if medication remains. The ADA report therefore limits its low-dose examples to eight weekly doses.

Calendar counting needs an explicit convention: 56 days equals eight elapsed seven-day intervals, but listing both day 0 and day 56 creates nine calendar dates. That arithmetic does not establish that a ninth injection is permitted. The labeled disposal date and the source’s stated eight-dose convention govern the table below.

Ignoring priming, overfill, and delivery loss, eight injections consume:
